Working within and outside of facilities, you will oversee the effective utilization of services and resources. In order to extend this high level of care, you will communicate with patients, families, other healthcare professionals, and social workers to promote patient safety, quality of care and cost-effective outcomes, for patients that require long-term care or home care after hospitalization. Minimum Requirements
  • At least 1 year of recent acute care experience in specialty
