Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Salem Hills Rehabilitation & Healthcare. As a LPN, you will provide daily support to our unit leadership and respond to resident and family needs within your scope of practice.

Key Responsibilities:

• Use the nursing process, judgment, and skills to set priorities and deliver quality direct resident care.
• Communicate and collaborate with the care team to revise and update the Nursing Care Plan.
• Monitor and document vitals, treatments administered, and residents' responses.
• Prepare and oversee the delivery of care for resident CNA assignments and activities.
  Requirements:
  
  • Graduate of an approved school of Nursing.
  • Licensed and currently registered in good standing in New York as an LPN.
  • Dedicated to providing extraordinary, clinically sound, and compassionate care.
  • Excellent communication, customer service, and organization skills.
