Job Summary:

Nursing Home Social Worker at Richmond Center for Rehabilitation and Healthcare is responsible for maintaining and improving residents' physical and mental/psychosocial needs. The ideal candidate will have prior long-term care experience and excellent communication skills.

Key Responsibilities:

• Maintain residents' ability to control everyday physical needs and mental/psychosocial needs.
• Identify individual social and emotional needs.
• Develop and maintain individualized care plans.
• Participate in the planning of residents' admission, community re-entry, or transfer to another facility.
• Participate in interdisciplinary care plan meetings to ensure treatment of residents' social and emotional needs.
  Requirements:
  • LMSW preferred.
  • Prior long-term care/sub-acute experience required.
